PRIVACY PROTECTION POLICY OF
ADVOCACY FOR PATIENTS WITH CHRONIC ILLNESS, INC.
In compliance with Connecticut Public Act 08-167
It is the policy of Advocacy for Patients with Chronic Illness, Inc. to protect the confidentiality of all personal information of its clients. Accordingly, we (i) protect the confidentiality of all Social Security numbers, driver’s license numbers and other personal information received by us; (ii) prohibit unlawful disclosure of Social Security numbers and similar personal information; and (iii) limit access to Social Security numbers and similar personal information. We make reasonable efforts to restrict access to Social Security numbers and similar personal information to those persons within our firm who need to know that information in order to complete work undertaken on behalf of our clients. Also, we make reasonable efforts to safeguard personal information obtained by us, including the destruction or erasing of computer files and other electronic media containing personal information prior to disposing of such computer files or other electronic media.
